CONSENTS TO PROCEDE WITH A DROP OFF TAX CLINIC APPOINTMENT
By registering with ACT’s Drop Off Tax Clinic, you must provide the following consents:
- Due to the conditions presented by COVID19, ACT is unable to offer the usual in-person assistance to prepare and file your taxes and benefits return. Instead, the service is being provided using alternative communication methods. In completing your tax and benefits return, we will be discussing information that is personal to you. The methods that we are using to communicate may not be secure. While all reasonable efforts have been made to ensure that your personal information is protected during the course of our communications, we cannot guarantee this.
- You consent to the use of a cell phone or cordless telephone for the purpose of discussions pertaining to completing your income tax and benefits return.
- You understand that your tax and benefits return will be prepared by one of ACT’s Community Volunteer who has been approved by the CRA. The Volunteer will prepare your income tax and benefits return electronically with the information that is supplied by you.
- You understand that our Volunteer is affiliated with ACT to provide this service to you. Our Volunteers are not agents or representatives of the CRA.
- If you fail to provide the necessary information to complete your return, we will not proceed further and your tax and benefit return will not be filed. We cannot guarantee a second appointment will be given.
- You will be notified by telephone once your tax and benefits return has been completed. We will arrange a mutually beneficial time for you to come to collect your documents. If your documents are not collected within 30 days, they will be destroyed.
- You understand that once your tax and benefits return has been submitted to the CRA, we cannot make any adjustments to your file. You must contact the CRA to resolve any issues.
I have read, understood, and agree to the above consents.
